The Office of Student Accessibility requests documentation of a diagnosed disability. Documentation must be current (dated within the past 4 years). Accepted documentation includes:
- A letter from a qualified evaluator that is certifying the diagnosis (Example: medical doctor, licensed psychologist, licensed counselor, etc). The letter should include their name, title, and contact information, and the diagnosis. The letter may include the student’s functional limitations and recommended accommodations, however this is not required and we cannot guarantee the recommended accommodations can be provided,
OR
- A diagnostic report that states the diagnosis, as well as the name, title, and contact information of a qualified evaluator. The letter may include the student’s functional limitations and recommended accommodations, however we cannot guarantee the recommended accommodations can be provided,
OR
- An IEP or 504 plan that lists the diagnosis as well as the name and title of a qualified evaluator and the name of the educational institution. You do not have to submit the entire IEP or 504, only the pages containing the diagnosis, the qualified evaluator’s name, title, the diagnosis, and the accommodations you previously received. We cannot guarantee that all previous accommodations can be provided.
